Do I need to bring anything to my initial visit?

Yes. If you downloaded and printed your initial visit paperwork, please fill it out and bring to your initial visit. Alternatively, you can email it to our Reception, or 15-30 minutes prior to your visit to fill out a hard copy.

Additionally, if you have medical records, including lab results and imaging reports, from the last 3 years, please bring them to your initial visit.
